Getting started is as easy as 1-2-3.

1. Simply give us a call at 602.258.8858 (outside Arizona call 800.318.0553) to receive the Clarifacts User Agreement.
2. Complete, sign and return the User Agreement by fax 602.258.7177 or mail to 130 North Central Avenue Suite 309 Phoenix, AZ 85004.
3. Once the agreement is received, we will provide you with the forms and letters needed to begin, as well as a user name and password for our Web-based system.
